I called this my HASH board.  For Hex Addressable Sample Hold.  As pictured, there are 0.001uF caps for the hold capacitors.  Those were probably way too small.  The thing never did work too well.  Changing them to 0.01uF would probably help a lot.  This card was set up so that four could be ganged up to provide DC control voltages to various things (64 total channels).
